You buy the salicylic acid cleanser. You use the retinol. You get the expensive facial. But the cystic acne on your jawline keeps coming back.
Why? Because hormonal acne is an “inside job.” No amount of topical scrubbing can stop a pimple that is being fueled by your ovaries.
The Villain: Dihydrotestosterone (DHT)
The primary driver of hormonal skin issues (both acne and unwanted hair) is a specific androgen called DHT.
- Your body makes Testosterone.
- An enzyme called 5-alpha reductase converts Testosterone into DHT.
- DHT is “Super-Testosterone.” It is 5x more potent.
- DHT binds to the oil glands in your skin, forcing them to pump out excess oil (sebum).
- Bacteria trapped in that oil cause deep, painful cysts.
The Signs It’s Hormonal
- Location: Primarily along the jawline, chin, and neck (the “beard area”).
- Timing: Flares up 7-10 days before your period (luteal phase).
- Type: Deep, painful cysts that don’t come to a head, rather than small whiteheads.
- Accompanied by: Hirsutism (dark hairs on chin/nipples) or hair thinning on the scalp.
Treating the Root (Androgen Excess)
To clear the skin, you have to lower the androgens.
1. Insulin Management
High insulin stimulates the ovaries to make more testosterone.
- The Fix: Low glycemic diet + Inositol supplements. Lower insulin = Lower testosterone = Clearer skin.
2. Spearmint Tea
A surprisingly effective anti-androgen. Two cups of organic spearmint tea daily has been shown to reduce free testosterone levels.
3. Zinc
Zinc inhibits the enzyme (5-alpha reductase) that turns testosterone into the acne-causing DHT. 30mg of Zinc Picolinate is a common therapeutic dose.
4. Dairy Evaluation
Cow’s milk contains IGF-1 (Insulin-like Growth Factor) which can spike insulin and acne in sensitive women. Try cutting only milk (cheese/yogurt are largely fermented and often okay) for 3 weeks to see if it helps.
