What ‘Unexplained Infertility’ Actually Means

“Unexplained Infertility.” It is the most frustrating diagnosis a couple can hear. It sounds like nothing is wrong, yet you aren’t getting pregnant.

Roughly 15-30% of couples seeking fertility treatment fall into this bucket. But “Unexplained” is a misnomer. It doesn’t mean there is no cause; it means the cause is currently undetectable by standard tests.

What Has Been Ruled Out?

To get this label, you must have already passed the “Big 3” checks:

  1. Ovulation: You are releasing an egg regularly (Progesterone test).
  2. Anatomy: Your fallopian tubes are open (HSG test).
  3. Sperm: The semen analysis counts and motility are normal.

The Hidden Causes

So what is missing? Usually, the problem lies in the microscopic steps that standard tests can’t see.

1. Egg Quality (Not Quantity)

You might have plenty of eggs (good AMH), but they might be genetically abnormal (aneuploidy) or lack the energy (mitochondria) to divide. This is common over age 35.

  • Solution: CoQ10 supplementation often helps mitochondria. IVF with PGT-A testing can diagnose chromosomal issues.

2. Sperm DNA Fragmentation

A standard sperm test checks “Are they swimming?” It doesn’t check “Is the DNA inside them damaged?” High DNA fragmentation is a major hidden cause of unexplained infertility and recurrent miscarriage.

  • Solution: Ask for a massive Sperm DNA Fragmentation Test.

3. Mild Endometriosis

As discussed in previous posts, “silent” endometriosis can create inflammation that prevents fertilization, even without pain.

4. Implantation Issues

The egg fertilizes, but the lining of the uterus isn’t “sticky” enough or the immune system attacks it.

  1. Lifestyle Audit: 3 months of aggressive egg/sperm quality improvement (No smoking, low alcohol, antioxidants).
  2. IUI (Intrauterine Insemination): Often the first medical step to bypass cervical mucus issues and get sperm closer to the egg.
